Gather Health SMS Communications Policy

Gather Health may use SMS (text messaging) to communicate with patients and care team members for care coordination and operational purposes.

1. Types of Messages

Patients
Patients may receive SMS messages related to their care, including:
- Scheduling and availability requests
- Care coordination and follow-up
- Responses to prior interactions or outreach

Care Team Members (Providers and Staff)
Care team members may receive SMS messages related to operational workflows, including:
- Alerts for patient-related triggers
- Task and queue notifications
- System-generated updates requiring follow-up

All messages are transactional and non-marketing.

2. Consent

Patients
Patients provide consent to receive SMS messages during enrollment, intake, or other interactions with Gather Health. Consent may be documented electronically, on paper, or verbally.

Care Team Members
Care team members provide consent during onboarding and system setup by supplying a mobile number and enabling SMS notifications.

Message frequency varies based on user activity, care needs, and operational events.

4. Message and Data Rates

Message and data rates may apply depending on your mobile carrier and plan.

5. Opt-Out

You may opt out of SMS messages at any time by replying:
